Open fractures often involve much more damage to the surrounding muscles, tendons, and ligaments. If a bone breaks in such a way that bone fragments stick out through the skin or a wound penetrates down to the broken bone, the fracture is called an open or compound fracture. In most cases, the number of bone fragments corresponds with the amount of force needed to break the bone. In this type of fracture, the bone has broken into three or more pieces. A twisting force to the thigh causes this type of fracture.Ĭomminuted fracture. The fracture line encircles the shaft like the stripes on a candy cane. This type of fracture has an angled line across the shaft. In this type of fracture, the break is a straight horizontal line going across the femoral shaft. The most common types of femoral shaft fractures include: Whether the skin and muscle over the bone is torn by the injury. The pattern of the fracture (for example, the bone can break in different directions, such as crosswise, lengthwise, or in the middle). The location of the fracture (the femoral shaft is divided into thirds: distal, middle, proximal).Femur fractures are classified depending on: The skin around the fracture may be intact (closed fracture) or the bone may puncture the skin (open fracture).ĭoctors describe fractures to each other using classification systems. The pieces of bone may line up correctly (stable fracture) or be out of alignment (displaced fracture). Femur fractures vary greatly, depending on the force that causes the break. ![]() 8/28/2023 0 Comments Comminuted open fracture pictures![]() ![]() Femur fractures in a child under 6 months are typically well-treated in a Pavlik harness or spica cast. No clear consensus has been reached regarding optimal treatment, despite attempts at the creation of formal guidelines.(Kocher, 2010) There are a number of trendswhich have emerged to help guide management based on the above characteristics. Treatment of femur fractures varies by age, fracture pattern, mechanism, weight of the child, and associated injuries. Open fractures are classified by the system of Gustilo and Anderson.(Gustilo, 1976) Transverse fractures are considered “length-stable” and some oblique or comminuted fractures are considered “length-unstable”. Stability of the fracture influences treatment. Level of the fracture on the femur is important, as displacement of the fracture pieces is characteristic based on the muscle attachment points and the forces applied to the fractured pieces. Fracture patterns include: transverse, short oblique, spiral, and comminuted. Plain radiographs are usually sufficient for establishing the diagnosis and for preoperative planning.įractures are classified by pattern, location, stability, and whether the fractures are open or closed. X-ray evaluation should include the whole femur, as associated injuries such as an ipsilateral physeal fracture about the knee or a femoral neck fracture can occur. Hemodynamic instability and significant drop in hematocrit are rarely present in a child with an isolated femur fracture and should alert the physician to look for associated injuries.(Ciarallo, 1996 Lynch, 1996) However, more subtle findings may be present in the small child. Typical physical exam findings associated with femur fracture include: deformity, thigh swelling, and pain. ![]() In the setting of suspected femur fracture, it is critical that the entire child is examined as associated injuries are common. Fifteen percent of femur fractures in children under 2 years and up to 80% of fractures before walking age are the result of non-accidental trauma.(Loder, 2006 Beals, 1983 Blakemore, 1996 Gross, 1983) Transverse fractures may be a better predictor of nonaccidental trauma in young children as compared to spiral fractures.(Murphy, 2015) In younger children, a fall is the most common mechanism. Falls and motor vehicle collisions are the most common mechanisms of injury, accounting for approximately two thirds of the injuries in older children. There is a bimodal age distribution of fractures, first in early childhood then again peaking in adolescence. Males more commonly sustain femur fractures, as they account for greater than 70% of injuries. Trauma is the leading cause of death and disability in children.(Waller, 1989 Haller, 1983 Peclet, 1990) When evaluated in the late 1990’s, orthopaedic trauma led to 84,000 hospital admissions annually and a cost of nearly a billion dollars.(Galano, 2005) Femur fractures were the most common reason for admission in this group. Surgical treatment has reduced the burden of care for families, shortened hospital stays, and decreased the early disability and disruption in the families’ lives.(Hughes, 1995 Karn, 1986 Kirby, 1981 Buechsenschuetz, 2002) Treatment varies by age, weight, and fracture patternįemoral diaphyseal fractures account for nearly 2 percent of all bony injuries in children, and are the most common orthopedic injury requiring hospitalization.(Sahlin, 1990 McCartney, 1994 Flynn, 2004) In the past, femur fractures in all children were commonly treated with immediate spica casting or a period of traction followed by casting.(Flynn, 2001) Non-surgical treatment with spica casting remains the standard for infants and toddlers less than 5 years however, school-age children are now more commonly undergoing surgical intervention.Adolescents have adult-like mechanism for femur fracture (high energy) and associated injuries are common. Most common femur fracture type in a child is closed, transverse, and non-comminuted.Femur fracture in a child before walking age is suspicious for non-accidental trauma. ![]() 8/28/2023 0 Comments Snappy lend reviews![]() There is a slight posterior lateral bevel that transitions quickly and efficiently into the midfoot. This creates a snappy toe-off that works best at moderate to high speeds. The carbon plate is most noticeable in the forefoot, where it adds a decent amount of stiffness. It is not super foam bouncy, but there is still plenty of resiliency that makes picking up the pace easy and fun. The new midsole is made up of two layers of FF Blast+, which provides a softer and far bouncier ride than the previous version. The Magic Speed 3 is a workout/racing shoe with a stiff carbon plate, bouncy midsole and a durable outsole. I had some mild abrasions on my foot using this for moderate efforts sockless, so socks are highly recommended. The inner mesh is a little scratchy and I would encourage anyone interested in this shoe to wear socks. The snug fit and good lockdown have kept my foot secure through all paces and turns. The security throughout the length of the shoe is quite good. There is a little more stiffness at the rearmost portion of the heel, but the counter is flexible and I had no issues. The heel features a thin amount of heel collar cushioning and a flexible heel counter. I did not have to tie the laces down tightly in order to get a secure fit and did not have to lace lock this shoe. The midfoot is snug and features a thin, gusseted tongue. The forefoot volume is low and tapered in the toe box. The mesh stretches a little but mostly holds its shape. For those who have wider feet, are between sizes or want to use these for longer efforts, I would highly suggest going up a half size. For short to moderate distances, true to size will fine if you are okay with a snugger/shorter fit. ![]() I found thin socks helped with the fit, which feels like how older/traditional racing shoes used to. The ride is surprisingly bouncy and snappy for a non-super shoe, featuring two layers of FF Blast+. The fit is snug and slightly short, reminiscent of older racing shoes. Is there ever room for teaching kids to memorize high frequency words? When we give then opportunities to sound out the words when reading them, we are providing an environment for orthographic mapping to take place. ![]() Instead, we should integrate those words into phonics lessons as often as possible.įor example, the Dolch primer list includes words like these:Įach of those words is perfectly decodable, and rather than teach them by sight, we should teach kids to read them using their phonics knowledge. ![]() We shouldn’t be sending home lists of 50 “sight words” for our kindergartners to master. I don’t think it’s wrong to teach kids to read words using those lists as a reference, but we need to approach it a different way. So what does this tell us about the big sight word lists … like Dolch and Fry? Then we teach learners to learn the tricky parts by heart. We call attention to the parts of the word that are phonetic (and there’s usually at least 1-2 of them). What about sight words that aren’t regular, like the? We teach them to sound out words, even sight words. We need to teach children to identify individual sounds in words and then connect those sounds to letters. In order for us to read words and then store them for future retrieval, we must be able to match the phonemes (sounds) to the graphemes (letters). Orthographic mapping is the process we use to store printed words in long-term memory. Stick with me … I promise it’s not as complicated as it sounds. ![]() How do we move from sounding out words letter by letter, to recognizing thousands and thousands of words instantly? It’s through a mental process called orthographic mapping. They do this so quickly and effortlessly that it takes a tiny fraction of a second to identify each word. Instead, they (very, very quickly) connect the letters to the sounds in each word. Researchers have discovered that strong readers do not call upon thousands of pictures of words in their brains. We are not trying to get students to cram pictures of words in their brains, because there’s a limit to how many words any of us can remember by sight. This doesn’t fit with how the brain learns to read. So should we get lists of sight words and get our students to memorize them using flash cards? A sight-word vocabulary refers to the pool of words a student can effortlessly recognize. ![]() However, reading researchers have a different definition of sight words.Ī sight word is a word that is instantly and effortlessly recalled from memory, regardless of whether it is phonically regular or irregular.
